Hey that's not Richard Crenna as Colonel Trautman! You're absolutely right so this is my guess of what happened. 
This is one of the many portrait ads Drew did to promote upcoming movies and at that stage in production,
Kirk Douglas was up for the role of Rambo's mentor and Vietnam Colonel. Due to a scheduling conflict
he ultimately couldn't commit himself to the movie, so Richard Crenna got the part.  Unlucky for him he didn't get
this great portrait, but then again he got a great part in a hugely successful film.
